Community Benefit Grants and Sponsorships

Bozeman Health’s mission is to improve community health and quality of life. We accomplish our mission, in part, by supporting community-based organizations through grants and sponsorships.

We would love to fund every application; however, we have a limited amount of money to distribute each year. Bozeman Health operates on a calendar fiscal year (January to December). Please review definitions, the applicant eligibility and requirements, and evaluation criteria carefully before applying.

About Community Benefit Grants and Sponsorships


The Community Benefit Grants process provides Bozeman Health with an opportunity to respond to requests and partner with eligible organizations whose work is aligned with our Community Benefit priority areas. Bozeman Health strives to support programming, education, access, and equity initiatives through Community Benefit Grants. Bozeman Health is only able to award Community Benefit Grants to organizations exempt under section 501(c)(3) of the Internal Revenue Code.

Grant applications that prioritize the top need identified by the 2020 Community Health Needs Assessments and the 2021-2023 Community Benefit Strategic Plans will be given priority. The current priority areas are:

  • Access to Health Care Services,
  • Nutrition and Physical Activity,
  • Mental Health, and
  • Injury and Violence Prevention.

2023 Grants Application

Applications are reviewed on a quarterly basis. The deadline for applications are:

  • Quarter 1: March 19, 2023 by 11am
  • Quarter 2: June 14, 2023 by 11am
  • Quarter 3: September 13, 2023 by 11am
  • Quarter 4: December 6, 2023 by 11am

The 2023 grant application includes 17 questions including 10 questions that are scored by the Community Benefit Grants Committee. The 10 questions are weighted based on the level of importance to the committee (such as 20% focused on our Community Health Improvement Plan strategic priority areas, 18% focused on helping us live out our Diversity, Equity, Inclusion, and Belonging commitments, and 10% a program evaluation reporting plan). Organizations awarded 2023 grants will be required to submit a report with program measures, monitoring, and reporting on performance.

After carefully reviewing the above information, please upload your grant documents to the following link by the deadline Be sure to attach the following documentation with your application, as applicable:

  • The application document saved as a PDF or Microsoft Word document
  • Copy of your Treasury letter certifying your 501(c)(3) tax-exempt status
  • Completed IRS form W9

We cannot review your application until all documentation has been submitted. The documents listed above must be included with your application.

The sponsorship process provides an opportunity for us to respond to requests from organizations whose work is in alignment with Bozeman Health’s Community Benefit and/or marketing and outreach priorities. Examples of appropriate sponsorship applications include requests related to community events and gatherings, conferences, health programs, print materials, etc.

2023 Sponsorships Application

Sponsorship applications are reviewed monthly by the sponsorship committee. After carefully reviewing the above information, please upload your grant documents to the following link by the deadline Please include a copy of your W-9 with the application at this link, which is required if we have not supported your organization in the past. If your organization is a non-profit, we will also need a copy of your Treasury letter certifying your 501(c)(3) tax-exempt status attached at the link above.
